Job summary

The Merthyr and Cynon Local Primary mental Health Support Service is excited to announce a vacancy for a band 7 Advance Practitioner.

The successful candidate will provide expert support to a well established team of mental health practitioners based across the Merthyr and Cynon locality.

The post holder will act as an advanced mental health practitioner within the context of a multi-disciplinary primary care service. They will offer specialist advice and deliver evidence based mental health care to clients in the GP surgeries and a range of community based premises and will play a major role in the continuous development of the service within the UHB; as well as working in close liaison with colleagues in our psychology department, outpatients, and Community Mental Health Teams, and will monitor referral rates to ensure that our community receive a timely and proportionate service and treatment plan designed to meet their individual needs.

Main duties of the job

To act as a highly specialist clinical expert, providing support and advice to GPs and other primary care providers such as practice nurses to enable them to safely manage and care for people with mental health problems

  • Ensure that a culture of effective assessment, formulation and positive risk management is present in the team.
  • Initiate and actively promote the development of evidence based practice.
  • Foster robust working relationships with all partners primary care, CMHTs Local Authorities and the voluntary sector.
  • To carry an assessment and intervention based caseload and take a lead on management of complex cases and supporting staff in management of such cases.
  • Promote advanced practice and the acquisition of new skills for the team and promote the profile of the Advanced Practitioner as an educator and expert practitioner.
  • Support and instigate the use of evidence based good practice through an evidence based approach using audit and promoting good practice.
  • Deputises for Team Leader as and when required.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

  • Relevant degree level qualification
  • Qualified Mental Health professional such as a registered Mental health Nurse, Social Worker, Occupational Therapist, Psychologist
  • Evidence of post- registration continuing professional development
  • Ability to demonstrate sound clinical knowledge and awareness of current evidence base relevant to the area of practice Sound working knowledge of the legal and policy framework underpinning mental health service provision e.g. Mental Health Measures, Mental Capacity Act, National Service Frameworks, Care Programme Approach
  • Knowledge of safeguarding procedures for children and adults
  • Evidence of achievement in clinical practice within the area of speciality
