An exciting opportunity has arisen for a Health Care Support Worker to join us in the Acute Clinical Team in a permanent position.
The post holder will work as a member of the Acute Clinical Team under the direction of the Team Leader.
The post holder will contribute to the implementation of nursing care as assessed and prescribed by the Clinical Practitioner/ Registered Nurse, undertake a range of delegated duties to meet the needs of specified adults in a variety of community settings.
This post includes working in a multi-disciplinary and multi-agency arena.
The range of responsibilities will be delegated by the Clinical Practitioner, Registered Nurse or Therapist to support patients within their own community home. The post holder will acknowledge their limitations and actively seek advice as necessary.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
An exciting opportunity has arisen for a Health Care Support Worker to join the Acute Clinical Team in a permanent position.
The post holder will work within the Acute Clinical Team which is based in Trem Y Mor, Bettws, Bridgend, which provides a comprehensive multi-disciplinary assessment service for adults presenting with complex needs in the community who are likely to be admitted to hospital in the absence of a timely health intervention.
The post holder will work closely with multidisciplinary team colleagues, contributing to the implementation of nursing care as assessed and prescribed by the Clinical Practitioner/Registered Nurse, undertake a range of delegated duties to meet the needs of specified adults in a variety of community settings.
We are looking for a skilled, caring professional with a commitment to excellence in nursing practice who will support the clinical leads with the monitoring of the Acute Clinical Team service performance and practice developments.
The Acute Clinical Team will provide a friendly and welcoming environment within a supportive management structure that promotes ongoing educational development.

- Knowledge an understanding of the Code of Conduct for Healthcare Support Workers in Wales.

- Recognises the basic of physiology, such as normal vital signs
