Job summary
The Advanced Nurse Practitioner (ANP) will support the ANP Lead in the review and development of the mental health outpatients' service provision across Mental Health in Cwm Taf Morgannwg University Health Board (CTMUHB) and act as a resource in instigating and providing clinical services.
The ANP will function as an autonomous practitioner and undertake ADHD assessments, utilising advanced clinical expertise to collect and collate past and current history. They will assess newly referred patients who have been discussed in the ADHD multi-disciplinary team.
The ANP will be expected to work within their level of competence and to work flexibly as part of the ANP outpatient team.
The role will include the requirement to have previously worked within the mental health speciality and to have relevant experience in working closely with both patients with ADHD and a wide range of professionals.
Main duties of the job
- Responsible for the delivery and management of designated ADHD assessment clinics.
- Assess and review patients' mental health presentation and needs, and recommend the most appropriate service that will aide recovery and achieve agreed outcomes, this may include transfer/discharge to another relevant service.
- Take responsibility for autonomous clinical decisions and assessments as appropriate within the realms of qualification.
- Co-produce, assess, plan and negotiate care with the patient and the patient's family and/or carers.
- Liaise as appropriate with Consultant Psychiatrist/other Health Care Professionals when the patient's care warrants escalation or de-escalation, further investigation or re-investigation.
- Provide or arrange physical examinations for patients.
- Hold and manage own caseload in line with Adult Mental Health legislation and best practice. This will include adherence to the Mental Health Measure and Section 117 guidelines and policies.
- Hold responsibility for a defined caseload of patients that only attend outpatient clinic and do not receive input from the wider Community Mental Health Team (CMHT).
- Ensure that clinic outcomes are disseminated to the relevant clinician involved in the patient's care such as the patient's GP, care-coordinator or practitioner in the Crisis Resolution and Home Treatment Team (CRHTT) or PCMHSS.
- Be responsible for providing accurate and concise patient records.
- Work as part of a multi-disciplinary team
This post is fixed term/secondment for 17 months to meet the needs of the service.
