Responsibility for the management of the patient / client in clinical settings. The post holder will be responsible for ensuring that all resources within that area are used to an optimum effect.
Participating in responsibility for ensuring that all policies and procedures are observed, and meet the Health Board requirements contained in the Health & Safety Policy.
Responsibility for administering medication in accordance with RCN guidance.
To respond to medical emergencies promptly and to ensure that emergency equipment is conveyed to the scene.
Participating in responsibility for the support and supervision of junior staff i.e. registered nurses, student nurses and health care support workers.
Be responsible for providing accurate, current, comprehensive and concise records concerning the condition of the client in accordance with the NMC Standards For Records and Record Keeping and in compliance with the Data Protection Act.
Ensuring that practice is in line with, and reflects the requirements of the N&MC Code of Professional Conduct, and is evidence-based.
Ensuring that any personal and professional development plans reflect the requirements of the NMC & the Post Registration Education and Practice guidelines.
Ensuring that Health Board Policies and Procedures are met in relation to Personal professional responsibilities.

- Liaise with the appropriate Clinical Specialists i.e. GPs, Practice teams and Clinical Nurse Specialists, in relation to individual patient requirements.

- Demonstrate good organisational skills with responsibility for ensuring efficient and effective use of human and financial resources.
The assessment, planning, delivery, evaluation and updating of individual plans of care, acknowledging personal beliefs, identity and preferences of parents/carers.
Undertake direct nursing care for patients across a range of procedures, who may require wound care e.g. removal of sutures
Enable patients to achieve physical comfort assisting in minimising client discomfort and pain.
Participate in basic health education of patients and carers.
Contribute to the ongoing support of patients and others significant to them, maintaining the patients interests, their identity and their well being.
Informing and advising the Nurse Manager/Deputy of actual or potential problems with regard to patients, staff, visitors, etc, and where necessary, contribute to the appropriate action in resolving such problems on an ongoing basis.
Be required to make decisions on care provided, making referrals as appropriate: these may involve judgements involving complex facts or situations, which require analysis, interpretation and comparison of a range of options, for example, when responding to a medical emergency
Assist in the supporting/teaching of other registered nurses, student nurses and healthcare support workers.
Support the practice based team in the organisation, facilitation and implementation of a comprehensive nursing service.
Complete any follow up action required following the consultation/treatment, ensuring this is both timely and appropriate.
Prioritise own workload to ensure effective time management.
Assist in the management of change where there is an identified need within the service.
Comply and support change, thus ensuring that any new practice is disseminated.
Be accountable for ensuring knowledge and skill is developed and maintained for any extended practice undertaken within an agreed protocol and procedure
Demonstrate good organisational skills and efficient use of resources when undertaking clinical activity.
Ensure the environment is safe for patients, meets the required standard of cleanliness and all equipment is maintained in good working order and checked as necessary
Manage and maintain a diary of all work activities, making it available for audit as necessary.
Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
Communicate with and support patients who are receiving bad news.
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
Utilise communication skills to support patients to adhere to prescribed treatment regimens.
Anticipate barriers to communication and take action to improve communication.
Estimate and maintain effective communication with individuals and groups within the practice environment external stakeholders.
Act as an advocate when representing the patients and colleagues viewpoints to others.
Liaise with colleagues in management of patients who require wound care within the clinic environment (including the application of prescribed dressings, suture removal etc)
Adhere to the manual handling policy and specific guidelines relating to bariatric patient in the clinic setting
Keyboard skills and information technology skills including word processing, email, intranet and internet, power point for communication, correspondence, clinical reports, teaching preparation, clinical literature searches
The post requires highly developed physical skills, where accuracy is important, but there is no specific requirement for speed, for example.
Undertake venepuncture procedures following appropriate training and on completion of care competencies.
Undertake ECG procedure following a period of training, reporting abnormalities to the GPs.
Be responsible for day to day supervision of staff, including clinical supervision and training which may include basic HR advice.
Be expected to assist in the identification and achievement of staff training and development needs through the implementation of Health Board policies and procedures.
Ensure a learning environment to encourage staff development.
Advise and arbitrate where there may be differences of opinion amongst staff.
Support the Nurse Manager/Deputy and staff when skill mix reviews are being undertaken.
Participate, where appropriate, in the review of job descriptions for more junior staff.
Participate in the recruitment and selection procedures.
Act as a mentor for student nurses.
Have the opportunity to develop, through the personal development review process their own clinical and managerial skills to the benefit of the clinical setting.
Ensure that staff working within the clinical setting are aware of their responsibilities within Health Board policies.
Participate in the Health Board Induction Programme at Directorate and Departmental level.
Ensure that HB and Statutory Policies and Procedures are followed and implemented, and report any problems including proposing changes to working practices or procedures.
At all times, assist the practice based team in the effective management of resources within sphere of responsibility.
Have an awareness of budget and spending trends within the Directorate, and associated skill mix/staff resources.
Where appropriate, will monitor and record expenditure e.g. medical and surgical, staff costs.
Follow Health Board/Statutory procedures in the safe storage of information and confidentiality.
Providing accurate, current, comprehensive and concise records concerning the condition of the patient, in accordance with the N&MC Standards for Record & Record Keeping.
Be required to access computerised and written information, which informs care e.g. care plans, observation charts investigation results (e.g. pathology and radiology.)
Be expected to participate in audit, supporting and implementing any necessary changes in relation to patient care.
Be expected to participate in research, interpretation and analysis of findings and their application to practice.
The post holder would be expected to provide evidence based care and have an active involvement in any research and audit within their sphere e.g. assist with audit of standards of care for wound infections.
