Job summary

An exciting opportunity has arisen for Band 3 Healthcare Support workers at HMP Parc. The successful applicants will be expected to work alongside the nursing team in a fast-paced and challenging environment. A certain degree of energy and resilience will be required as the role involves independent working the ability to react to change with flexibility and enthusiasm.

Main duties of the job

The health care support worker will work as part of the nursing team within standard operating procedures, and Registered Nurses and General Practitioners are available for reference.

The post holder will assist with specified delegated clinical tasks that include the provision and monitoring of Chronic Disease management providing a caring, patient focused service at all times.

The post holder will deal with queries from patients, Health Care professionals and other agencies face to face and by telephone. The role will also involve administration and clerical duties including updating patient records on the computer system.

The post holder will work as a member of the Primary Care Nursing Team under the direction of the lead nurse.

The Health Care Support Worker will support Registered Nurses, General Practitioners and other health care professionals in the assessment, treatment and management of patients presenting in primary care settings.

Job description

Job responsibilities

  • Carry out delegated clinical duties within HMP & YOI Parc.
  • Undertake Phlebotomy clinics and dry blood spot testing.
  • To perform physiological measurements such as blood pressure monitoring, pulse monitoring, waist circumference measurement, weight monitoring.
  • Providing any necessary Health Promotion advice to patients and carers e.g. Smoking Cessation, Weight Management Support, Alcohol Consumption.
  • Undertake Spirometry testing, and record result in patient information.
  • Undertake ECG monitoring.
  • To participate in the administration of immunisations under a Patient Specific Directive following a period of training i.e. B12, influenza and pneumococcal injections.
  • To perform basic wound management under the supervision of a Registered Nurse following appropriate training and assessment.
  • To perform suture/clip removal under the supervision of a Registered Nurse following appropriate training and assessment.
  • To offer advice to patients and carers on a variety of health issues, signposting individuals to sources of help and specialist advice to facilitate behaviour change.
  • Following training and assessment, undertake Point of Care Testing, ensuring appropriate quality control measurements are carried out.
  • To complete requisition forms for medical, surgical, paper and pharmacy store requests under the supervision of the Registered Nurse/GP.
  • Provide administrative support for chronic disease management screening in various health care settings, recording test results in patient records.
  • Contribute to the screening and assessment of patients in line with agreed Protocols and Guidelines.
  • Work effectively with all members of the Primary Care Nursing Service and interact with patients and other health care colleagues in a professional manner.
  • To instruct patients on inhaler technique to patients with Asthma/COPD, following a period of training and assessment.
  • Inputting data onto the appropriate database.
  • Assist primary care teams in maintaining up-to-date registers of patients with chronic disease.
  • Undertake various administrative and clerical duties deemed necessary e.g. answering telephone, faxing, photocopying.
  • To promote high standards of patient care, demonstrating duties to more junior colleagues and less experienced staff.
  • To ensure the effective use of all resources, materials and equipment paying particular attention to the safekeeping, cleaning, decontamination and storage of equipment.
  • To prepare the clinical environment for the patient ensuring the treatment area is clean, stocked and tidy.
  • Act as a chaperone when the situation arises.
  • Perform urinalysis and record results in patient records.
  • Following a period of training, assist the Registered Nurse/GP in a Minor Surgery Clinic when required e.g. dermatology procedures, ENT clinic.
  • Act as the patients advocate clearly presenting the patients opinions to others.
  • Attend emergency codes when requested and provide support to clinical and operational team.
  • To have knowledge and be aware of responsibilities under the Assessment, Care in Custody and Teamwork Process in Prison (ACCT) act within the prison environment.

The post holder will ensure patient confidentiality, privacy and dignity at all times, using a range of verbal and non-verbal communication tools.

The post holder will communicate factual information with patients, parents, guardians and carers who may have difficulty in understanding or communicating e.g. may be dysphasic, depressed, have auditory/visual impairment, have learning difficulties, do not have English as a first language or may be unable to accept diagnosis.

The post holder will :

  • Encourage patients/carers to adopt an active approach to regaining and maintaining their personal independence. This will require empathy, sensitivity, tact, persuasion and good interpersonal skills.
  • Work and liaise with the Primary Care team as well as patients/relatives whilst adhering to all University Health Board policies and procedures.
  • To report any change in the patients condition/environment to the Registered Nurse or other members of the multi-disciplinary team in a timely and professional manner.
  • Contribute to the development of patient care by attending relevant team, department or other appropriate meetings and commenting on recommended changes to clinical care or service provision.
  • Arrange and confirm appointments for annual review/follow ups/treatment by telephone, post or face to face invite. This will also include re-arranging patient appointments at short notice due to unforeseen circumstances such as cancelled clinics, which will require negotiation skills.
  • Respond appropriately to patient queries, often dealing with anxious and distressed patients/carers face to face or by telephone.
  • Contribute to the protection of all vulnerable adults and children and report any concerns to the Team Leader as per Safeguarding Adults and Childrens policies and procedures.
  • Promote equality for all individuals to include promoting anti-discriminatory practice as well as supporting individual rights and choice within service delivery.
