Job summary
We are seeking a specialty doctor to join the Frailty team at Ysbyty Cwm Cynon to deliver excellent and safe care for our patients. This appointment will be for up to 12-month fixed term initially.
The post holder will support and help drive forward developments, improving the quality of care delivered to patients with a focus on maximising peoples' independence and supporting their return to their usual place of residence wherever possible
Our aim is to provide excellent care for frail older patients from community to hospital and back home. We aim to treat acute illness appropriately, to enhance quality of life, to reduce disability, recognise when life is nearing its end and to provide individualised person centred care in the right place at the right time.
The successful post-holder would support the development and clinical transformation of our frailty pathway and will be required to support the delivery of outpatient and inpatient services for the local population and in doing so foster excellent working relationships within and outside the specialist area, including medical, nursing, secretarial and support staff, General Practitioners and Management Teams.
The post will provide support to the on-call rota which may be subject to change, in accordance with appropriate consultation process with post holders.
Main duties of the job
THIS POST IS FIXED TERM FOR UPTO 12 MONTHS DUE TO MEET THE NEEDS OF THE SERVICE
You will provide a clinical service which will include the responsibility for the prevention, diagnosis and treatment of illness. There is opportunity to develop your clinical skills with support from COTE Consultants.
You will be required to adhere to the principles of good medical practice laid down in the guidance of 'Good Medical Practice' issued by the General Medical Council.
You will be required to participate in the Cwm Taf Morgannwg University Health Board's risk management process and in clinical audit and other clinical governance activities within the department, the Clinical Service Group and Cwm Taf Morgannwg University Health Board. Adherence to Caldicott Principles is mandatory. The post holder will be positively encouraged to participate in research and development in line with Cwm Taf Morgannwg University Health Board's Commissioner and NHS priorities.
Travelling as necessary between hospitals/clinic will be required to undertake such duties.

Teaching
The Department has a high clinical teaching workload in both postgraduate and undergraduate spheres which the post holder would be expected to contribute towards.
Audit
The Clinical Service Group identifies 4 afternoons per year for presentation of audit work. The Successful candidate will be expected to take part in the department audit activities.
