Job summary
We are looking for an energetic and enthusiastic member of staff to join the Pembrokeshire Urgent and Intermediate Care Team. This is an exciting development opportunity to work in an operational role, with a forward-thinking team.
You will be working closely with the Clinical Lead Nurse Patient Flow, and Clinical Lead GP Intermediate & Urgent Primary Care to progress developments with our Porth Preseli model, including Hospital at Home.
Main duties of the job
The post holder will act as a key member of the Urgent and Intermediate Care team, supporting service development and operational management.
The post holder will operationally support the day-to-day running and service development of the Pembrokeshire Care Hub (Porth Preseli). This supports timely and effective patient access to urgent and intermediate care across the county.
To achieve this, the main duties will be to:
- Provide leadership and coordination for urgent response, care hub, community hospital beds, and patient flow.
- Deliver on key performance targets, quality, and safety standards.
- Drive service innovation using telehealth, telecare, and digital technologies.
- Represent the service at the county, Health Board, and national levels.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.
