Job summary
The postholder will work as part of the Children's Disability Health Team to deliver safe, compassionate and effective care and advise to children and young people in home, educational and residential environments. They provide delegated personal and clinical care under the supervision of a registered healthcare professional and contribute to maintaining a safe, supportive environment for children and families.
They carry out delegated personal and clinical care duties with minimal supervision, in line with HEIW All Wales Delegation Guidelines, and ensure the supervising registered professional is kept informed of any changes in the child or young person's condition, progress, or functional ability.
Main duties of the job
Assists with assessments, care plans and reviews with preparation of information packs, making appointments and organising clinics or home visits under the direction of the Specialist Nurse.
Gives appropriate advice on child related situations i.e. nutrition / fussy eating, sleep difficulties, sensory difficulties, toileting, bowel and bladder difficulties.
Undertake home visits to children as determined by the Specialist Nurse and support under their supervision
To carry out ongoing assessments of a child's care needs and report to the child's including behaviour, socialisation, financial support, childcare support, housing, language barriers, parenting, physical and mental health issues, disabilities and learning difficulties and report back to Specialist Nurse.
Follow up of non attended appointments i.e. from therapists / other support agencies with families.
Assist in completing referrals as advised by the Specialist nurse to therapists / other support agencies
Attend and contribute to team meetings, clinics and referral meetings.
Organise own schedule and planning of day-to-day tasks and planning of diary for scheduling children and family visits, clinics and school visits.
Assist in the liaison with other agencies, i.e. social services in planning and supporting the child's needs/
Support schools with training required appropriate to role.
