The Urgent and Intermediate Care team have an opening for a dynamic and experienced individual to join our Discharge Specialist Team. The team support in both community and secondary care settings to facilitate timely hospital discharges and enhance the quality and safety of the service given to individuals at home with an aim to optimise recovery and resilience.
The Discharge Specialist Team is an adaptable team with a variety of functions to support and advocate the patient in the interface between services and organisations, identifying the appropriate pathway for the patient, ensuring 'what matters to them' is at the forefront of decisions in the process.
The service aims to deliver support for discharge to recover and assess at home working in conjunction with our community multidisciplinary team and in-reaching into our hospitals.

Main duties of the job
We are seeking an experienced Senior Nurse to lead our Discharge Liaison Team across Withybush General Hospital, South Pembrokeshire Hospital, and Tenby Cottage Hospital. This Band 7 role is pivotal in ensuring safe, timely, and person-centred discharge planning for patients with complex needs. You will provide visible clinical leadership, expert advice, and coordination of care packages across health and social care, working closely with multidisciplinary teams and community services.
The successful candidate will demonstrate autonomy, advanced assessment skills, and the ability to manage competing priorities in a fast-paced environment. Strong communication and negotiation skills are essential, alongside a thorough understanding of discharge-to-assess models, continuing healthcare processes, and patient flow strategies. Experience in leading teams, managing change, and working across primary and secondary care is highly desirable.
