Job summary

An exciting opportunity has arisen for a Full Time 37.5 hours per week, Band 7 Team Leader within the Continuing Care Team within the Carmarthenshire area.

This role will allow for consistency in management which will be able to provide the appropriate level of assurance level required that staff have the skills and expertise to provide competent, confident and compassionate care to patient with complex health and Palliative and End of Life Care (PEOLC) needs.

We are looking for a motivated candidate with significant previous Community Nursing and staff management experience, to lead community services into the future.

Main duties of the job

The successful candidate will be responsible for the management, supervision and teaching of a team of band 3 Health Care Support Workers in addition to a Band 6 operational Registered Nurse.In addition to this they will provide clinical advice to the team and other professional agencies, acting as a resource for evidence-based care. This will include additional Band 7 management tier to provide an oversight of the third sector provision of PEOLC within the County to ensure appropriate prioritisation, allocation and provision of care.The successful candidate will provide clinical leadership and work as part of the MDT, in order to meet the needs of the caseload and local population and must be both an experienced clinician and manager of staff.The ability to travel between locations in a timely manner is essential for this post.
