Job summary

Your role will involve:

- Set up and deliver autonomous led clinics, including face to face and virtual. Where required for the role, utilising advanced assessment skills which may include clinical assessment and diagnosis competencies and skills.

- Acting as a practice leader, managing their own workload and work across professional, organisational and system boundaries to improve services and develop evidence-based practice.

- Improving the management of patients by optimising investigations and treatments which may include initiate, titrate and or prescribe medication and therapies, working within current Health Board and profession specific guidance and regulations.

Main duties of the job

You will be required to:

- Autonomously deliver a caseload of patients with highly complex needs, using evidence-based, client centred principles to critically analyse, assess, plan, implement and evaluate interventions. This will include education, health promotion and support on all aspects of the patient's treatment and management plan.

- Work collaboratively with other members of the multi-disciplinary team across different pathways and care settings to ensure the health care needs of patients are met. This will include continual evaluation of the current service and reflection on own practice.

- Act as an expert resource to other health professionals across a number of settings, monitor and support services, and participating in pathway development, education research and audit, whilst continually developing and updating knowledge and highly specialist skills required for the role.
