Health Care Support Worker - Frailty Support Worker

Job summary

An exciting opportunity has arisen for a Band 3 Frailty Support Workers on Sunderland Ward, South Pembrokeshire Hospital in Pembroke Dock.

We are looking for a compassionate and motivated frailty support worker to join our dynamic multidisciplinary team (MDT).

Can you make a meaningful difference in the lives of older adults living with frailty or are needing additional rehabilitation by providing direct support and improving their quality of life?

The post offers the opportunity to develop your skills and experience in caring for frail patients & Rehabilitation. Frailty is a growing challenge within healthcare, and we, as healthcare professionals are at the forefront of providing the dignity, support and care that people deserve.

The aim of the role would be to improve the patient experience. You would be expected to promote the patients' independence by supporting in ward activities with a focus on mobility, personal hygiene, nutrition and hydration in order to maintain and maximise their functional ability.

Main duties of the job

The post holder should hold or be willing to complete a NVQ level 3 in Health or equivalent.

Previous experience in frailty, rehabilitation, & elderly care.

They will need to demonstrate that they have good communication skills and that they are an excellent team player as well as being able to work alone. They will be expected to work alongside both the nursing and the therapies team.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
