Job summary

Pembrokeshire Intermediate Care Team are offering an exciting opportunity for a motivated, enthusiastic and passionate Band 3 NursingSupport Worker to join our Care at Home team.

The successful candidate will be competent to work without direct supervision, within a framework that has been agreed, to provide high quality care to patients including end of life, therefore reducing the need for acute hospital admission and facilitating earlier discharges.

Main duties of the job

As part of the Care at Home Team you will ensure that patients receivesafe, individualised and effective standards of care by following care plansand treatment interventions as directed by the designated Coordinator.

The successful candidate will need to communicate effectively with otherteam members at all times across all of the integrated teams anddemonstrate a flexible approach to the needs of the service.

Community experience is desirable, but not essential as induction andtraining will be provided.

The ability to travel in a timely manner across the Health Board is essential.

The ability to speak Welsh is desirable for this post; English and/or Welshspeakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

Essential

  • Level 3 qualification in a health care related subject or equivalent experience and can evidence work based competency demonstrating theoretical knowledge, and the ability to undertake clinical care duties relevant for the role which is gained through experience and worked based training/short courses
  • Knowledge of the Code of Conduct for Healthcare Support Workers in Wales and All Wales Delegation Guidelines (HEIW)
  • Knowledge of relevant policies and procedures within a healthcare setting for example the principles of Safeguarding adults/children to keep patients within their care safe, and confidentiality for example General Data Protection Regulations (GDPR)

Experience

Essential

  • Experience of working within a relevant heath or social care environment, providing direct patient care

Desirable

  • Experience of working with patient groups relevant to area of practice
  • Experience of working within the NHS

Language Skills
