Job summary
The Assistant Practitioner (Nursing Support Worker) role is designed to deliver evidenced based clinical care under the direction and support of a Registered Nurse by following the Health Board's protocols, policies, procedures pathways and care plans.
The post-holder will be responsible for the delivery of a high standard of safe, individualised, efficient, cost-effective care under the delegated authority of a Registered Nurse.
The post-holder will communicate effectively at all times and in conjunction with other ward/team members, contribute to and maintain the ward/team philosophy.
The post-holder will practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.
The post-holder will develop the role as required within the boundaries of the KSF post outline and agreed competencies.
Main duties of the job
Undertake identified allocated duties in relation to the care environment, the direct care of the patient/client as identified by a Registered Nurse and in accordance with Hywel Dda Health Board policies and procedures.
Perform duties and activities within the framework of a plan of care, as set and reviewed by a Registered Nurse at regular intervals.
Assist patients/clients to achieve physical and emotional well-being and comfort.
Ensure that any changes in patients/client's condition or circumstance are reported immediately to the appropriate person/Registered Nurse/nurse in charge.
Utilise transferable skills within level of competence.
The CCAP will have received training and education to undertake additional duties and will have completed the in-house advanced training programme for the Critical Care Associate Practitioner.
The CCAP will provide essential nursing care for patients categorised as requiring Level 1, 2 and 3 care - with an identified qualified nurse as a supervisor.
The training will allow the CCAP to assess, plan, implement, and evaluate the care they've provided to the allocated patient alongside their supervisor (Registered Nurse), who ensures that support and advice is available for any aspect of care that is not within the realms of the post holder's capabilities / responsibilities.

- Level 4 vocational qualification in a relevant health care associated subject / Certificate in Health Care Support Worker Education
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
- Evidence of continuing personal development relevant to the role in health care
- Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and the all Wales Band 4 Assistant Practitioner Governance Framework.
- Clinical knowledge which is relevant to the role.
- Demonstrates understanding of the importance of following procedures and treatment plans
- Demonstrates an understanding of the legislation related to Safeguarding e.g. adults/children, The Mental Capacity Act and Deprivation of Liberties Safeguards to keep adults within their care safe
