This is an exciting opportunity to work at the forefront of clinical pharmacy within a fast-paced, patient-focused environment. You will be embedded in the multidisciplinary team, supporting safe and effective medicines use from the point of entry into hospital care through to supporting the transition of patient care back into the community.
You will be part of the exciting rollout of electronic prescribing and medicines administration (EPMA) within the specialist area, supporting colleagues across the MDT, as we embed digital systems within our practice. You will utilise innovative digital tools to prioritise and share patient care across the interface.
Main duties of the job
Your responsibilities will include:
- Providing expert clinical pharmacy input to patients in admissions(ED) and those in the clinical streaming hub and SDEC.
- Utilising your independent prescribing qualification to support medicines reconciliation, optimisation, and discharge planning.
- Providing wider clinical pharmacy service within the hospital.
- Collaborating closely with medical, nursing, and allied health professionals to ensure seamless patient care.
- Contributing to service development and quality improvement initiatives.
- Supporting the development of current and future pharmacy professionals, as well as other healthcare professionals, through clinical mentorship.
