Job summary

As part of this role, you will perform clinical assessments and play a key role in vaccine administration, preparation and post monitoring. You will develop skills in venepuncture, cannulation, IV/IM treatments and screening.

You will work alongside a range of services, including Community Outreach, working with hard-to-reach service users. You will support specialist teams including blood borne virus, school nursing, primary care, public Health, local authority, Occupational Health, Infection prevention and control, midwifery and children's services.

You will be at the forefront of health protection and immunisations, responding appropriately to outbreaks, testing and post exposure vaccinations. You will make a direct and lasting impact on the health and well-being of our Hywel Dda community. The role is predominantly community based, with the flexibility to support outbreaks in secondary care settings. Whether you are vaccinating/testing in homes, schools, GP clinics, community pop-up venues, you will be a valued member of the team.

Main duties of the job

Work with service leads to analyse information gathered through contacts with families and undertake assessments and make informed decisions and professional judgments on treatment schedules and pathways which must be recorded in the documentation and when writing reports.

Work with the service leads to assess families with complex needs which will inform referrals to other multi agency professionals if necessary.

Engage with colleagues, multi-agencies and relevant stakeholders to target areas of low uptake of immunisations and outbreak scenarios and improve and enhance uptake across the whole Hywel Dda University Health Board's footprint.

Plan, organise and prioritise own workload and identify and discuss competing priorities service leads.

Act in accordance with legal and statutory guidance and comply with all Hywel Dda University Health Board policies and procedures, including safeguarding.

Act independently within the health board and professional guidelines, referring to supervisory management as appropriate.
