Job summary
This an exciting opportunity to join the Care of the Elderly team at Glangwili General Hospital. This new Consultant appointment will focus on providing an Elderly Care and Frailty service at the "front door" of the hospital, with expertise being concentrated in SDEC, the Emergency Department, the Clinical Decision Unit and Medical Assessment Unit. The successful candidate will work in close conjunction with other Consultant Physicians in the hospital and the community.
Main duties of the job
o To provide a service in Elderly Care (Frailty)/General Medicine including supporting the development of the COTE service.
o Work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing hight quality healthcare.
o To possess experience and knowledge in running a sub specialty service.
o To provide treatment of medical patients presenting to GGH front door including frailty, Emergency Department, Clinical Decision Unit and Same Day Emergency Care.
o Providing effective leadership to multidisciplinary staff members in the CDU, SDEC and ED
o Provide clinical supervision of the SDEC and Emergency Department medical liaison patients.
o Ensure close cooperation with other medical consultant colleagues to develop seamless pathways of care for patients.
o Work with the team to the development of "hot clinics" as an alternative to admission.
o Ensure close liaison with GP colleagues to direct patients to the most suitable service.
o Ensure development, support and training for nurse consultant and nurse practitioner colleagues.
o Participating in CPD including audit.
o Acting as an Educational Supervisor to Resident and Middle Grade medical staff.
o To participate in the acute general medical on call at Glangwili General Hospital.
