The role is an important one, as you will be using your knowledge, experience and expertise to provide support to hard-working frontline staff within the District Nursing service of Fishguard and St David's localities.

We have 1 permanent vacancy available, 0.9 whole time equivalent which equates to 6 and 3/4 hours a day over 5 days a week. The District Nursing service covers 8am - 6pm 7 days a week, including bank holidays.

Main duties of the job

You will be a key member of the Fishguard and St David's District Nursing Team, which provides care for patients in their own homes.

The coordinator role is varied, it can often be challenging when multiple tasks need actioning at once, the role requires a high standard of communication, documentation and initiative. As a team we are expected to be kind and responsive, professional and informative and contribute to the quality of our services by demonstrating the behaviours detailed in our Values.

You will receive queries via telephone, email and face to face conversations and will resolve issues using your expertise of pathways and processes. To be successful in this role you will need excellent time management skills, must be able to work on your own initiative, under pressure and be able to prioritise your own workload with minimal supervision.

The role is diverse, including taking referrals, managing IT systems such as Civica, Allocate, Outlook and Excel. You will be working with a number of different professionals in a busy, supportive environment. Some knowledge of health and social care services would be beneficial as you will be dealing with diverse referrals and pathways, gathering information and coordinating responses.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services
