Job summary
An exciting opportunity has arisen for an experienced and committed Nursing Support Worker to join the Carmarthenshire Community Nursing Service, working within the Continuing Care Team. It is a County wide post, caring for patients living within the 3 localities of Carmarthenshire.
Working as part of the team, within Community Nursing services, you will be responsible for providing high quality care to patients in their own homes living with life limiting, palliative or terminal illnesses.
Main duties of the job
The post holder will ensure high standards of individualised care are delivered to patients and their families and carers within the home environment, by following care plans and treatment interventions as delegated by the registered nurse, as well as having the ability to work competently without direct supervision. Carmarthenshire Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.
This is a permanent position worked over a 7-day period including weekends and bank holidays. Full time and part time hours available.
The post holder will need the ability to travel between sites in a timely manner.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
