Community Mental Health Practitioner (Older Adults)

The opportunity has arisen to work within the Community Older Adults Mental Health Service in beautiful West Wales.Based in North Ceredigion .

The successful applicant will be responsible for coordinating (CTP) a case load as part of a multidisciplinary team.

Main duties of the job

The post holder will be responsible for the assessment of care needs for older adults experiencing mental illness both functional and organic, the development of programmes of care (CTP) and the implementation and evaluation of these programmes of care for a defined clinical caseload.

We are looking for practitioners who are passionate about promoting the well-being and rights of older people, interested in contributing to service development and maintaining a high standard of clinical care.

You will have considerable opportunities for training and professional development and regular clinical supervision will be available.

Full time hours are preferred (but part time hours considered).

The ability to speak Welsh is desirable for this post: English and /or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.
