Job summary

An exciting opportunity has arisen In Hywel Dda University Health board for a band 7 Specialist Clinical Practitioner for Frailty. The role will be based in the Llanelli locality and the base for the role will be at Eastgate Offices in the heart of Llanelli.

Eastgate is a very busy open plan office that is integrated and houses a multitude of health, social care and voluntary care agencies.

The role will involve attending weekly MDTs and managing your own caseload of complex patients who are frail. You will be expected to travel between bases and to and from patients/clients' homes.

Main duties of the job

Support the prevention of unplanned hospital admissions, identifying and initiating Comprehensive Geriatric Assessments where required. andsetting up management plans in collaboration with the patients, carers and community integrated team, to enable the patient to stay at homewhere possible, adopting a home first approach.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and knowledge

Essential

  • Registered Nurse, Level 1 (active NMC registration), Registered Allied Health Professional or Health Care Scientist (active HCPC Registration).
  • Studied at Master's Level or have experience at equivalent level.
  • Proficient level of knowledge in clinical speciality
  • Knowledge of service improvement
  • Teaching and assessing qualification or equivalent experience
  • Up to date working knowledge of both local and national policy issues pertinent to adults with Frailty syndromes i.e. legislation
  • Evidence of continuing professional development in a relevant area of specialist practice

Desirable

  • Demonstrable clinical assessment and history taking skills
  • Relevant Masters level degree
  • Relevant specific qualification, e.g. Assessment of the Older Person Module

  • Proven experience of working with older adults / patients with frailty / complex case management including the development of clinical management plans
