Job summary

We have an exciting opportunity in Hywel Dda University Health Board for a Therapy Assistant Practitioner in Falls. The successful candidate will support the Community Falls and Frailty Therapy Service to deliver specialist therapeutic and rehabilitative intervention as directed; contributing to the Health Board's aim of service delivery closer to home for individuals with ongoing needs following a fall or physical / functional decline.

The successful candidate will cover Ceredigion by participating in the Falls Safe & Steady clinics at Aberystwyth, Aberaeron and Cardigan. Therefore, travel to these sites is expected. The Therapy Assistant Practitioner will work closely with other members of the MDT. We are looking for an enthusiastic, motivated individual who has the ability to motivate and engage with their patients.

Whilst the role focusses on Falls, we require an individual who is adaptable to meet needs of patients with multiple and a variety of conditions as well as service priorities. Due to the nature of the role you will need to be able to travel on daily basis within the County, as well as being flexible to travel across the Health Board on occasions.

Main duties of the job

To support the Ceredigion Falls Safe & Steady Service to deliver specialist assessment and rehabilitative intervention as directed; contributing to the Health Board's aim of service delivery closer to home supporting individuals who may present with a range of diagnoses and require input from more than one therapy service.

The post holder will be involved in undertaking detailed observations, assessments, following/modifying therapy plans and promoting an enabling approach to recovery. The service can be delivered in individual or group settings within homes, clinical, care facilities or community settings according to need and following agreed guidelines, protocols, policies and care pathways.

Aspects of the role will include completion of relevant administrative duties and lone domiciliary working. The provision of a flexible 7-day service may be expected of this role, based on service need.
