We work together to provide early intervention, period of rehabilitation and assessment to reduce the risk of loss in function, risk of deconditioning, nutritional deficit, deterioration in their mental health to increase people's time spent at home rather than hospital.

Our approach is to deliver patient care in a holistic, person-centred manner, supporting patients, when possible, to stay/return in a safe manner to their own homes where they may continue to be a valued member of their own community. This would included outreaching into community settings.

This role involves supervising students, junior members of staff, rehabilitation assistants and therapy support staff (assistant practitioners and support workers).

You will work alongside acute and community colleagues and services to create a joined up rehabilitation pathway and positive experience for our population.

This role could be part of a rotational opportunity between Front of House, stroke & general rehabilitation within Prince Philip Hospital.

Successful applicants will require an excellent ability to work autonomously, in high pressure and fast paced environments, managing and prioritising a diverse and unpredictable caseload, while keeping the HB values central to everything they do.

As a versatile postgraduate, you will need to have strong clinical grounding in older people rehabilitation, expertise in frailty syndrome, stroke, neurological conditions, cardio respiratory conditions, MSK problems and a good understanding of current issues affecting acute, intermediate care and community services.
