Job summary
The Acute Response Team based within Intermediate Care are pleased to be offering this exciting post as part of our Home First approach across Carmarthenshire.
The Health Care Support Worker role has been developed within our Home First Team to provide short-term, intensive rehabilitation at home for patients to enable them to return home from hospital sooner, providing access to care 24 hours a day / 7 days a week.
The post will deliver delegated care across a range of disciplines as part of a team-based approach to delivering holistic, patient-centred care. The key aim of the role is to support people to achieve independence through the delivery of an individualised plan of care as determined by the team.
We are looking for an individual who is motivated to make a positive contribution to the lives of the people we support and have a "can do" approach to their work.
Community experience is desirable, but not essential as full induction and training will be provided.
The successful candidates will need to communicate effectively with other team members at all times across all of the integrated teams and demonstrate a flexible approach to the needs of the service across the 7 days.
Main duties of the job
The Team offers-
Support and advice from the multidisciplinary team for patients and carers.
Identified goals to increase independence with functional activities following discharge home. This may include goals with washing, dressing, functional mobility, meals, or any goals identified by the MDT.
Continuation and promotion of exercise programme if prescribed by a physiotherapist.
Support to carers on initial discharge by supporting the patient with functional tasks.
As one of our Heath Care Support Workers, your duties will include working side-by-side with OTs, Physiotherapists, Nurses, GPs, Dieticians and other health and social care professionals, undertaking delegated tasks and providing planned care and support to people that will help them to remain in their own homes.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
