An exciting opportunity has arisen for enthusiastic Health Care Support Workers to join a new dynamic Frailty Team within Puffin ward at Withybush General Hospital.

Frailty is a rapidly growing specialism that aims to reduce hospitalisation for frail and older patients in Pembrokeshire. This is a developing service and we are searching for motivated and enthusiastic individuals.

Main duties of the job

We are a supportive, dynamic and emphatic team optimising our Health Board values at all times.

As a pathway we focus on a robust Multi-disciplinary approach to ensure patients have the best possible outcome whilst staying with us. The MDT are enthusiastic about improving the hospital experience for people who are frail and need a short stay in hospital.

Our ambition is to deliver excellent local and specialist services, to improve the health and wellbeing of our patients, and provide a vibrant place for staff to develop, innovate and build careers.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

About us

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
