We have an exciting opportunity available to work alongside our Clinical Lead Nurses and Professional and Practice Development Nurses, supporting the improvement of quality and implementation of change, facilitating learning and development, and ensuring staff are supported via coaching and supervision.

This position will involve working across the three counties with base to be dependant on location of successful candidate.

This is a fixed term contract to cover a secondment and approval for secondment will need to be agreed prior to interview.

Main duties of the job

The post holder will lead on empowering the Community Clinical Lead Nurses and District Nurse Team leaders to deliver a well-defined, well managed and responsive nursing care in the community.

They will:

Provide senior coaching and mentoring support to Community Clinical Lead Nurses and District Nurse Team leaders across HDUHB.Lead the development of District Nurse (DN) team leaders within Hywel Dda University Health Board (HDUHB), utilising tools such as advocacy, connecting to resources, experiential sharing, relationship building, group facilitation and skill building/mentoring/goal setting.Identify opportunity for changes in practice which meet the needs of the local population and lead on opportunities for the team members to develop and excel as part of a DN team and as part of the wider multi professional team within a neighbourhood.Provide restorative supervision based on the A-Equip model to Community Practice and Professional Development Nurses, Clinical Lead Nurses and District Nursing Team Leaders using a 1-1 or group approach.Evaluate with Community Practice and Professional Development Nurses, DN Team leaders and teams the value of the A-Equip model.Oversee and lead the existing Community Practice and Professional Development Nurses in establishing standardised practices across HDUHB.
