Job summary
Hywel Dda University Health Board are currently looking to recruit a suitably qualified and motivated individual who has an interest in working with older adults experiencing mental illness both functional and organic to join the Pembrokeshire Older Adults Community Mental Health Team based at Bro Cerwyn, Withybush General Hospital, Pembrokeshire.
Various full and part time hours will be considered.
The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload. The post holder will also be expected to undertakethe role of a Care Co-ordinatorwithin the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
Essential to the post is the ability to work in collaboration with the multi-disciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface.
Main duties of the job
The opportunity has arisen to work within the Community Older Adults Mental Health Service in beautiful West Wales.
Based in Pembrokeshire.
The successful applicant will be based Pembrokeshire and will be responsible for Co-ordinating (CTP) a case load in the north or south of the county and will be part of a multidisciplinary team, co - located with the Older Adult Mental Health unit at St Brynachs.
The service works with older adults with both functional mental health issues and / or dementia. We are looking for practitioners who are passionate about promoting the well- being and rights of older people, interested in contributing to service development and maintaining a high standard of clinical care.
The ability to speak Welsh is desirable for this post: English and /or Welsh speakers are equally welcome to apply.
