Job summary
This post comes during a period of Service Transformation in response to the changing needs of people with a learning disability and the national agenda for improvement. If successful, you will play a vital part within this exciting period of change, by contributing to the design of a new service model providing flexible & responsive services.
The post holder will;
- Work under the direction of the Band 6 Health Liaison Nurses (LD) in providing a health liaison service to people with a learning disability who are admitted to hospital.
- Work in collaboration with acute and secondary care providers to ensure reasonable adjustments are made in delivery of care to people with a learning disability and their carers.
- Be expected to support training provided to acute services staff under the direction of the Band 6 Nurse.
- Directly assess patients referred to the service from acute and secondary care and provide advice and guidance on making reasonable adjustments to care for the patient in relation to behaviours that challenge, changes to environment etc.
Main duties of the job
The main duties of this role are;
- To provide guidance on the health needs of people with a learning disability under the supervision of the Band 6 Nurse.
- To input into the development of care pathways to reduce health inequality faced by people with a learning disability locally and attend national events in respect of the health needs of people with a learning disability.
- To participate in research and audits carried out by the team.
- To disseminate and share good practice and model high professional standards in their own practice.
- To input into the identification of barriers to access and develop action plans to eliminate these barriers in partnership with acute and secondary care staff.
- To participate in education programmes for staff in the acute and secondary sector.
- To input into the development of health promotion initiatives across HDUHB.
- To work closely with colleagues in community learning disability teams across the Health Board.
- To participate in planning and providing training and support sessions to meet identified needs making use of all available resources and based on current research and knowledge.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
