Job summary

Hywel Dda Health Board are currently looking to recruit Community Mental Health Nurses to join the Community Mental Health centre based at Haven Way Day Hospital in Pembroke Dock , Pembrokeshire.

This role covers the areas of South Pembrokeshire, including Narberth and surrounding area, Neyland, Pembroke and Pembroke Dock , ( and surrounding areas) , Saundersfoot and Tenby.

Hywel Dda are in the process of transforming our mental health services. In Pembrokeshire the CMHT are now co-located with the Pembrokeshire CRHT and will be working towards a joint approach to patient care with the aim of developing a single seamless service to meet the needs of our patients on an individual basis with in the CMHC.

Main duties of the job

Our services work through an assessment & recovery approach with people accessing secondary mental health services for severe and enduring mental illness.

The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload including the role of a Care Co-ordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.

The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies. The role with cover 7 days per week and will require some weekend working.
