Job summary

An opportunity has arisen to join the HDUHB Primary Care Nursing Team on a part time basis (15 hours/week) joining the current post holder leading the professional and clinical nursing agenda across primary care nursing services.

The key purpose of the post of Head of Primary Care Nursing is to provide leadership, strategic direction, management and professional support to Primary Care Nursing Teams across Hywel Dda University Health Board.

This post will direct, lead and motivate staff within the Primary Care setting to enhance the patient experience ensuring the delivery of a high quality nursing service that is evidence based and is fully aligned to health board strategies and plans.

This role will support the Director of Primary Care, Community and Long Term Care in the achievement of strategic and operational performance targets working within financial resources.

Main duties of the job

The post holder will:

Work with the Director of Primary Care, Community and Long Term care and be professionally and clinically accountable for the delivery of effective and efficient Primary Care Nursing Services.

Work within Health Board guidelines and ensure compliance with all statutory assurance and governance requirements.

Engender a professional culture within their own area of work, communicating the agenda and engaging staff and other stakeholders to ensure that all components of the defined role are understood, delivered and embedded across the organisation in line with statutory requirements.

Provide professional leadership and direction for all registered and unregistered nurses working in Primary Care Nursing Practice Development Team, Primary Care Nursing Teams in Hywel Dda University Health Board managed practices, Out of Hours Nursing Team and Independent Primary Care Practices.

Ensure the Health board's aims and objectives and associated targets and strategies are clearly communicated to all relevant staff so that effective contributions to implementation of operational plans can be made.

Provide leadership in the management of change and promote empowerment to effect individual and team ownership of change in the course of implementation.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
