We are looking for an enthusiastic and dynamic Dietitian to join our well respected community dietetic team in Ceredigion as a clinical lead for community and Integrated care. This post is a fixed term, 1 year opportunity and offers an ideal opportunity for a Dietitian looking to develop their clinical leadership skills or an experienced clinical lead that would like to specialise in community dietetics. The post-holder will fully embed themselves as part of a wider MDT, supporting embedding nutrition/hydration as part of models of care and driving forward innovation. The post-holder base will be confirmed and we welcome applicants who would like to discuss remote and flexible working opportunities.

Main duties of the job

Acting clinical dietetic lead for the community integrated care team, working collaboratively with proximal community multidisciplinary services to strengthen communication and clinical delivery across community settings.

Autonomously manages a varied and complex caseload of adult dietetic referrals with nutritional support needs working as part of the community team and wider multi-professional team to provide specialist dietetic advice in relation to nutritional management and to produce a coordinated care plan of care. Leads on embedding and strengthening nutritional screening at the 'front door' and as part of rapid response pathways and models of care including providing training and education, including accredited training to therapy assistant practitioners and others working as part of the front door model to de-escalate risk

Supports the wider community nutritional support team by providing operational leadership alongside other community leads, ensuring effective prioritisation of referrals and that dietetic pathways and processes are fully embedded.

Delivers dietetic care in a variety of settings to meet the needs of the patient including undertaking home visits, attending care home/residential settings, community hospitals and providing group education and working across virtual platforms.
