Clinical Educator On The Ground Education Project Lymphedema

The on the ground education for lymphoedema is that! We work collaboratively with community staff to raise awareness of lymphoedema and update staff with regards to managing patients with chronic oedema and wet legs. We promote education as work-based learning, giving community staff the tools and education to treat patients with chronic oedema and wet legs efficiently, effectively, and safely.

We travel across the entire health board to work with district nursing teams throughout. Collaborative working between Lymphoedema, the cellulitis team, GPs' and other health care professionals to improve patient's outcomes and quality of life.

Main duties of the job

The position requires a Level 4 Certificate in Healthcare.

An exciting opportunity has arisen in Hywel Dda University Health Board for motivated and enthusiastic health professionals to be part of the Lymphoedema team, working as Lymphoedema Community Clinical Educator.

This is a part time post.

This is an exciting opportunity for skilled, compassionate and innovative practitioners to assist the Clinical Lymphoedema Lead to deliver the On the Ground Education Programme within an integrated Health Board model. This post is open to registered Nurses and Allied health professionals.

The successful candidates will work closely with the Clinical Lead in Lymphoedema in order to evidence and meet value based outcomes for the service.

A key responsibility within this role is to support the implementation of the "on the ground education" for Lymphoedema management including the Wet Leg Pathway across the 3 counties of Hywel Dda Health Board.

The post holders will be expected to work closely with peers and colleagues across the organisation in order to influence change and improved value based outcomes within our target population.

There will be continued positive support and close liaison and working arrangements with the National Lymphoedema team as part of this role in terms of continued professional development.

The post has a Health Board wide responsibility for implementing the "on the ground" educational programme.
