An exciting opportunity has arisen for an enthusiastic pharmacist to join the Medicines Management team, working in Primary Care covering the North Ceredigion geographical area, supporting patients who have attended A & E / CDU in BGH (in anticipation of a Same Day Urgent Care department being set up locally).

In this innovative role, you will be responsible for undertaking follow up medication reviews in primary care for patients identified during attendance at Bronglais Hospital A & E or CDU as needing an in-depth review. The aim is to improve compliance, reduce risk and de-prescribe where appropriate in order to ensure safe and effective medicines optimisation in line with the Prudent Healthcare principles.

Main duties of the job

You will need excellent clinical and communication skills and have a confident and professional manner in order to liaise effectively with a wide range of healthcare professionals. You will work collaboratively with secondary care colleagues to aid medication-related queries in patients recently discharged from hospital, thus providing seamless cross-sector integrated care. You will also work closely with community pharmacy colleagues ensuring that recent changes to medications post discharge or medication review are effectively communicated in a timely manner

The ability to work autonomously and collaboratively to ensure patients with complex pharmaceutical needs are reviewed effectively and relevant clinical interventions highlighted to GP's in a timely manner is a must. You must be highly motivated and skilled at influencing to be effective in the role.

This post is ideal for a Pharmacist who likes to work both independently and as part of a large team and who enjoy variety in their daily work.
