Job summary

Pembrokeshire Community Nursing Services are looking for a dynamic, enthusiastic Band 5 nurse with particular interests in the areas of palliative care, rehabilitation and care of the elderly to join our multidisciplinary team on Sunderland Ward at South Pembrokeshire Hospital.

Sunderland Ward is a 40 bedded ward in a community hospital providing care to patients with often complex and challenging health and social care needs. As a nurse led ward you will be supported to work autonomously and gain skills in a variety of clinical areas including cognitive impairment/dementia, frailty, palliative and end of life care needs, complex wound care management and rehabilitation, working closely and interactively with multidisciplinary teams. You will also develop your leadership and management skills with complex case management and care coordination skills, along with the opportunity to become link nurses for specialist services and areas.

With the introduction of the Social Care and Well Being Act (Wales) 2016 our nursing ethos very much puts the patient firmly in the centre of their care planning and you will be expected to work closely with patients, families and carers, specialist services, multidisciplinary teams in caring for patients with a variety of health and social care needs

Rotational options will be considered and available within community and acute services; interest in this exciting opportunity can be discussed at interview.

Main duties of the job

The successful applicant will assist and support the Senior Sister to provide clinical, managerial and professional leadership of Sunderland Ward and to be accountable for the assessment, planning, implementation and evaluation of nursing care for a caseload of patients within the ward. You must be innovative in the approach to care and be able to accept change. You will be responsible for the ward in the absence of the Senior Sister/Sister.
