Job summary
This is an exciting opportunity to be part of the development and implementation of the Health Boards Intermediate and Urgent Primary Care model. You will be joining a growing cohort of similar clinicians and intermediate care multidisciplinary team who are passionate about providing holistic person-centred care and managing risk to support people in their own homes. You will be working as part of the wider system with similarly minded clinicians across acute, include SDEC and Frailty, OOH, WAST, Primary care working together to ensure a joined up integrated coordinated response with a Home First ethos.
For interested candidates there is the additional opportunity to take on a broader leadership and development role including responsibility for scoping new pathways, developing SOPs and building key relationships with acute, OOH, WAST, Primary Care and community colleagues. For the successful candidate(s) interested in this leadership role there will be the opportunity to negotiate an additional sessional uplift (on the salaried GP scale) to salary.
Main duties of the job
The appointment of the right people to expand and enhance the team will allow us to realise our forward-thinking initiatives. Our vision is to have seamless, patient-centre care across our geographical area, irrespective of where the patient is currently residing consistent with the individual patients wishes, and any advanced care planning.
This role is pivotal in providing clinical leadership, innovative thinking, building cross organisational/services/team relationship, challenging the status quo to move our home first agenda into the future.
- Support the scoping, development and delivery of our enhanced community/virtual ward model
- Engage with a wide range of clinical stakeholders for the management of patient on the virtual ward
- Provide clinical leadership to the wider multidisciplinary team
- Work at the interface between acute care, acute frailty and existing primary and community services
- Support of the development of pathways that support admission avoidance and / or early discharge particularly for frail older patients.
- Shape the role of the urgent and intermediate care MDT and response
- Provide mentorship and support to advanced practitioners and other urgent care clinicians
- Clinical triage with Welsh Ambulance Services NHS Trust (WAST) and mitigating conveyance to acute hospital where appropriate.
- Management of patients in step up/step down beds to support recovery, rehabilitation and timely discharge as part of our intermediate care response
