Hywel Dda University Health Board has a high performing, energetic, Value Based Health Care Programme (VBHC), focused on organising our services around the outcomes that matter most to people.
The VBHC Programme underpins Hywel Dda University Health Board's long term plans as set out within its Healthier Mid and West Wales strategy.
We are looking to recruit an experienced and highly motivated Project Manager to join the Value Based Health Care team. This is a Fixed term contract for 12 months.
The post holder will have responsibility to project manage and deliver the objectives of the VBHC programme, ensuring that the agreed areas of focus are supported through the lens of Value.
You will need to have a flexible approach to work and welcome change and progression.
This is a fixed term/secondment post until 30/06/2024 to meet the needs of the service. If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.

Main duties of the job
Set-up, manage and implement projects within the VBHC programme, working closely with clinical leads and within partnership structures and governance.
Scope and develop a robust project plan for the Value Based Health Care workstreams in conjunction with key stakeholders.
Manage the project set up, implementation and reporting.
Develop the measurement of impact and tracking of benefits to the clinical teams and partner organisations, and lead on the reporting and feedback within Hywel Dda University Health Board to support programme development.
Communicate to, and gain support from, internal and external stakeholders.
Ensure robust measurement, supporting the development of outcomes measures across Hywel Dda University Health Board to inform programme boards of progress.
Working with the Senior Value Business Partner and finance team, develop analytical models on outcomes and costs across complete pathways of care, working closely with clinical teams and other professionals (especially patient outcomes, finance, and costing teams) to support delivery of Value Based Health Care projects.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.
