Job summary

An exciting opportunity has become available for a Band 6 Senior Mental Health Practitioner, GP Clusters, who will provide supervision to designated Well-being Practitioners across the counties of Carmarthenshire, Ceredigion and Pembrokeshire.

Individuals will be expected to be enthusiastic and committed to ensuring that the team rapid access well-being and mental health support to help address locally identified need and sustain capacity within wider Primary Care Mental Health Services. You will work autonomously, providing high quality complex clinical triage and clinical assessment to individuals presenting with mental health illness/conditions, in mental distress or acute crisis, resulting in a safe and appropriate clinical decision.

Work base will be discussed at interview stage.

The ability to travel between sites across the counties in a timely manner is essential.

Main duties of the job

The post holder will use a range of communication methods adaptable to the needs of individuals, families, carers and partner organisations to review mental health telephone enquiries through assessing symptoms and health information, prioritising for urgency.Provide clinical support and act as a decision maker for team members and work to improve the quality of the triage assessment, to enable the best possible care outcomes for those experiencing mental health distress, families/carers and partner organisations.
