An exciting opportunity has arisen to join the patient flow team as a Care Transfer Coordinator. This post will be offered as a secondment or fixed term until March 31st, 2027, and based in Hereford County Hospital. The applicant will work in collaboration with the multi-disciplinary / multi agency team to facilitate the seamless transfer of patients from nominated District General Hospitals directly to own home or to Community Hospital, Residential Home or Nursing Home.
Main duties of the job
The post holder will actively participate in relevant meetings to support flexible working and effective service provision. They will work in full compliance with the NMC Code of Conduct or HPC/professional standards, escalating any concerns appropriately. As a professional role model, the post holder will consistently demonstrate high standards of conduct and ensure these standards are upheld within the wider team.The role includes providing professional advice to nursing and therapy colleagues and working as part of the Care Transfer Coordinator team to compile and distribute the daily bed state to key managers and stakeholders. The post holder will identify, manage, and minimise clinical risks, supporting the Senior Manager for Unscheduled Care, Senior Nurse for Unscheduled Care, and Lead Therapist in developing plans and strategies informed by national direction and clinical need.
