Job summary
The purpose of the post is to ensure the provision of seamless, integrated services for individuals requiring complex/long term care, within the remit of Mental health and Learning Disabilities. The post holder will be an expert in the Continuing NHS Healthcare assessment providing advice and leadership to the clinical teams to guide them through the process ensuring all patients have appropriate assessment and consideration for on-going NHS support to meet their needs.
Main duties of the job
Key elements of the role include expert patient assessment to inform the development of individualised personal plans of care and collaborative, interagency working across professional and organisational boundaries.
Knowledge and understanding of Mental Health Act (1983) amended 2007, inclusive of S117 aftercare responsibilities and The Mental Health Measure in Wales 2010 in relation to the provision of Mental Health services and the role of commissioned care for the PTHB resident
The role will also work closely with the bed management process for out of county commissioning pathways for Adults of Working Age to ensure timely and prudent interventions are in place for these individuals.
To work within the boundaries of the local arrangements Powys Teaching Health Board acute commissioning pathway to assist in repatriation and monitoring for Out Of County placements via the acute pathway, supporting decision-makers with the identification of appropriate, on-framework care settings.
The role is pivotal to partnership working between health and social care for individuals with complex needs. They will actively drive forward the integration of Powys Teaching Health Board and Powys County Council Social Services and support the implementation of the National Framework for Continuing NHS Healthcare within a defined geographical area of Powys and for patients from that geographical area.

- Post Registration specialist training / supplementary qualifications to degree/ masters level in related subject/ or demonstrable equivalent experience

- Knowledge of current Nursing issues within NHS care systems
