The post holder will be part of our team of Diabetes Nurse Specialists delivering a comprehensive, high quality evidenced based Diabetes Specialist Nursing service to a designated population within Powys in collaboration with the Multi-disciplinary Teams across Primary, Community and Secondary Care along integrated pathways of care. They will actively contribute to the planning and development of the Diabetes services across Powys within the team of Diabetes Specialist Nurses and provide training and development to PTHB staff and partners within the Community Resource Team on diabetes management, treatment and care.

Main duties of the job

We are looking to recruit a motivated forward thinking Diabetes Specialist Nurse to join our team in Powys Teaching Health Board. We have an exciting opportunity to join our established diabetes nursing team providing a high standard of care to residents in Powys.

Our Diabetes Team is small, friendly and passionate about providing the highest possible standards of care. We have 22.5 hours available to provide diabetes specialist nurse cover across the county. The vacant posts will predominantly cover South Powys. Our service covers in-patient, out-patient and community, providing both virtual and face to face consultations. This role offers a unique place to work for a practitioner wishing to progress their career as a diabetes nurse specialist.

We are a friendly team and a team who would support anyone new to our service. We are looking for a team player who has experience and knowledge in diabetes and is motivated to develop the service further. Providing an advanced level of practice that encompasses aspects of education, research and management but is firmly grounded in direct care provision or clinical work with patients, families and populations. The role requires the successful applicant to work at an advanced level, promote public health and well-being and to understand the implications of the social, economic and political context of healthcare.

  • Degree in Nursing or health related subject
  • Post Registration specialist training / supplementary qualifications to Masters level in related subject (Diabetes Speciality)
  • Teaching qualification/ Teaching and Assessing in Clinical Practice or evidence of teaching skills/ clinical educator
  • Knowledge and skills to manage patients with complex needs

Desirable

  • Independent Prescribing Qualification
  • MSc Advanced Clinical Practice

  • Experience at management level demonstrating ability to deliver improvements to service users care
  • Experience of clinical assessment and proactive caseload management of respiratory patients
