Job summary

Powys Children & Young People's Health Services are pleased to be recruiting a new, blended senior nursing role to a team that is transforming the way it delivers care for Children & Young People in Powys.

The Lead Advanced Nurse Practitioner for C&YP is a blend of both advanced clinical practice and senior leadership so that the benefits of the Enhanced, Advanced and Consultant practice framework, can be fully realised.

The role will work across traditional boundaries of health and social care and of neighbouring secondary and tertiary care systems, ensuring that the holistic needs of children and young people are met for those with short term enhanced health needs and those with Chronic and long term health needs. The role as such is expected to be approximately 50% clinical with the remaining 50% awarded to the pillars of, Leadership, Strategy , Training, and Research. The role is responsible for line management and ensuring effective clinical governance is met for other specialist and advanced practitioners within the services and for the development of new roles/pathways according to assessed need.

The successful candidate will be able to evidence experienced advanced clinical practitioner knowledge and skills with evidenced strategic leadership, innovative practice and people management skills. The successful candidate will also need to be an Independent non-medical prescriber as part of the role. For the right candidate, annex 21 would be considered.

Main duties of the job

The main duties of the job include:

Holding a clinical caseload of children and young people with complex needs in the community,

Nurse led clinics, requiring a high level of assessment skills, ordering tests and investigations as necessary and interpreting results.

An Independent non medical prescriber, supporting safe prescribing in the community for a defined caseload

Co-ordination of those Children & Young People who require commissioned services to meet their need and ensuring transition points between services are seamless including that between children and adult services

Lead on clinical audit within services and develop initiatives to improve clinical pathways, utilising PROMS/PREMS

Lead Safe service development towards transformation of community delivered care, liaising with the Senior Leadership Team and Organisational colleagues as required

Development of a sustainable staffing model to support enhanced, advanced and consultant practice to support children and young people, Line managing an identified core team of practitioners, ensuring safe systems of care.
