We have an exciting opportunity for a Band 7 Advanced Practitioner Team Lead to join the Community Respiratory Service. You could be a Nurse, Physiotherapist or Occupational Therapist.
We're looking for a practitioner with proven leadership skills to support the Clinical Lead, providing operational support to the MDT based in North Powys. Our multidisciplinary team is provides:
- Management optimisation and review of patients with chronic lung conditions,
- Symptom management, ongoing review and support for patients with end stage respiratory disease who may be approaching end of life,
- Self management education and health advice/information to patients, relatives or carers,
- Supported discharge,
- Admission avoidance,
- Community clinics,
- Oxygen assessments and review for patients requiring oxygen at home,
- Pulmonary rehabilitation
Our transformation continues to provide respiratory services closer to home through a multidisciplinary team approach. We want a leader who uses their clinical expertise and advanced practice knowledge to provide safe, high-quality, value-based, patient centred respiratory care to Powys patients. The successful candidate will need to be able to communicate with a wide range of people, including patients and their families, other health care professionals and external agencies, including district general hospitals outside of Powys.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
Work as an Advanced Practitioner in Respiratory Long-TermConditions.
Demonstrate effective leadership creating an environment where staff treat individuals and each other with dignity and respect, there are high levels of trust, and empowerment, underpinned by effective communication, collaboration and partnership working in the field of long-term condition management.Lead the development of appropriate pathways for people with Respiratory LongTerm Conditions to ensure that they can access appropriate services to support their recovery and self-management.Direct, lead and motivate staff to ensure a high standard of professionalism, efficiency and effectiveness in service delivery, ensuring that activity is fullyaligned to the plans and strategies of the Health Board.Use recognised organisational development training (OD) techniques tocontribute to the development of the clinical and managerial aspects of theservice.Work with colleagues in Workforce and Organisational Development and Senior Managers across the Health Board in the development and delivery of effective and efficient development programmes for the areas within their remit.Contribute to ongoing learning, education and development strategy whichsupports the delivery of a competent workforce at all levels across the Health Board. Acts as an adviser in the identification of development needs within specialist area and contributes to the achievement of delivery of appropriate development.
