Job summary
An exciting opportunity has arisen for a Band 6 Nurse to join the Specialist Palliative Care Team within Powys Teaching Health Board based in Newtown
The role will provide a valuable learning experience regarding all aspects of specialist palliative care. The successful post holder will need to be flexible to meet the needs of the service, they will be based in Hosbis Hafren Newtown and will form part of the team providing specialist palliative care to the residents of Powys.
We are a rural Health Board with 133,000 people living across an area that is a quarter of Wales. With no District General Hospital we provide as many services as possible locally through GP provision, Community Services, Community Hospitals and Clinics.
The Specialist Palliative Care Team comprises of Palliative Medicine Consultants, Clinical Nurse Specialists, Community Palliative Care Nurses and Secretaries. The team provides specialist palliative care within the community, supporting palliative patients within their own home, care homes and during admissions to our Community Hospitals.
Main duties of the job
To deliver a comprehensive, high quality evidenced based palliative carenursing service to a delegated caseload of patients within Powys, incollaboration with the Multi-disciplinary Teams across Primary, Community and Secondary Care along integrated pathways of care.
To play a role in the provision of training and development to Powys Teaching Health Board (PTHB) staff and partners within the Community Team on palliative care.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
