This role is for a fixed term of 6 months initially - working mainly in the South of Powys across a number of environments including home, school and short break provision , as part of a Multidisciplinary team supporting Children and Young People with learning disability and complex /challenging needs, through the delivery of high quality, evidence based, behavioural assessment and interventions.
Main duties of the job
To provide a high quality, community-based LD nursing service to the children, adolescents and their families who are referred to the service. This will include the assessment of each referral to identify clinical need, priority, service provision and whether involvement with other disciplines is appropriate. Ensure a person-centred approach to all assessments and planning and that wherever possible, the families of the referred child or adolescent are included in the assessment and ongoing management of the cases. This will include the use of a range of interventions and approaches i.e., behavioural, cognitive individual and family-based approaches.

Provide a high quality, community-based LD nursing service to the children, adolescents and their families who are referred to the service. This will include the assessment of each referral to identify clinical need, priority, service provision and whether involvement with other disciplines is appropriate.Ensure a person-centred approach to all assessments and planning and that wherever possible, the families of the referred child or adolescent are included in the assessment and ongoing management of the cases. Provide specialist assessments of children and young people referred to the team based upon the appropriate use, interpretation, and integration of complex data from a variety of sources including self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with clients, family members and other stakeholders.Formulate and implement plans for the treatment and/or management ofbehaviours that challenge stemming from behavioural models and employing methods based upon evidence-based interventions and their efficacy, across the full range of care settings that the individual accesses.Responsible for implementing a range of behavioural interventions forindividuals, carers, and groups. Produce Behaviour Support Plans (BSP) in accordance with NICE guidance.Exercise autonomous professional responsibility for the assessment, treatment and evaluation of children and young people whose presenting behaviour are managed by a Behavioural Support Plan working alongside other professionals in the multidisciplinary team, and within the statutory fHealth Act, Mental Capacity Act, and the Mental Health (Wales) Measure.Provide specialist advice, guidance and consultation to multi-disciplinary teams.Undertake risk assessment and risk management for individual clients on the caseload and to provide advice to other professions on aspects of riskassessment and risk management. Ensure that all clinical plans demonstrate an adequate account of risk assessment and management.Act as a culture carrier within the service, promoting person centred values and a person-based approach to learning disabilities, reducing restrictive practice and behaviours that challenge in all settings.Provide expertise, advice, and support to facilitate the effective and appropriate provision of behavioural management by all members of the supporting team.Undertake skills training and delivering presentations to a wide range ofprofessionals, parents, and carers.Lead presence within the multi-disciplinary team to actively contribute to the development of Childrens LD Services.Foster close working relationships with all appropriate agencies in the field of LD locally, regionally, and nationally.Ensure that each accepted referral of child and young person receives anindividual specialist assessment and is the subject of regular evaluation which,wherever practicable is formulated in conjunction with the child and/or their family.Provide specialist interventions for referred children and their families, within a therapeutic framework, both individually, with families and in groups.Act as the key worker for an identified caseload and liaise with Care Managers across Social Services.Participate in regular clinical supervision sessions with an appropriatesupervisor with expertise in behaviour analysis and managerial supervision with the Children Service Manager.Learning Disability service presence within the Neurodevelopmental Service.Contributing to the diagnostic assessment process for children and youngpeople with social and communication difficulties
Co-ordinate and liaise with other team members to ensure adequate service provision.Ensure effective channels of communication are maintained creatingopportunities for the dissemination of all relevant information to staff within the defined area.Communicate effectively with Womens and Childrens staff and partners,ensuring that their views and wishes are heard, informing decisions whereappropriate.Foster close working relationships with all other health professionals, partners and external agencies to co-ordinate health and wellbeing care in the best interest of the child and their families.Contribute to service provision and development by communicating effectively with all PTHB personnel and multi-agency partners within and outside the Directorate
