Job summary

Adult Mental Health -Locality - Newtown, Powys

The post holder will work within an established, integrated, multidisciplinary team, focusing upon individuals with severe and enduring mental health problems.

Responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as definedwithin Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

Within their role as Care Co-ordinator the post holder will facilitate thepromotion and maintain a high standard of nursing care adhering to currentlegislation, professional and corporate guidelines.

Main duties of the job

Work within Powys Teaching Health Board policies and procedures. Facilitate User and Carer involvement in all aspects of service delivery. Assist the Team Manager with the management and organisation of their clinical area. Responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.

Ensure that the (Wales) Mental Health Measure (2010) legislative standards are adhered to and met. Assess and manage the care of clients with complex risk behaviours.

Responsibility for formulating and developing standard and complex, outcome focused care and treatment plans. Co-ordinate and facilitate, in conjunction with the care plan, appropriate resources (e.g. Welfare Rights, Housing Agencies, Voluntary Agencies, Day Services etc.) Ensure effective lines of communication are maintained with Inpatient. Monitor and evaluate commissioned services utilised within the care packages.
