The service enables clients recovering from a cardiac event to achieve their optimal health potential and empower them to choose and adopt the appropriate long term lifestyle changes.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
We are looking for a motivated, enthusiastic, individual to join our Cardiac Specialist Nursing Team Powys.
Main duties of the job
You will be working with a team of Cardiac Nurse Specialists who are currently based in the North and Mid of the County and provide specialist assessment and management for a designated cardiac population of Powys. We need somebody to deliver a comprehensive, high quality evidenced based Cardiac and Heart Failure Specialist Nursing and Rehabilitation service in collaboration with the Multi-disciplinary Teams across Primary, Community and Secondary Care.
The Cardiac Nurse Specialists provide a specialist nurse led community service to educate, support and advise patients with cardiac conditions through home visits, clinics in Community Hospitals and Medical Practices; providing telephone support and Cardiac Rehabilitation programmes. Using expert knowledge of managing and delivering care for patients with highly complex health needs within the community setting, ensuring the delivery is safe, efficient and effective. Reviewing the effectiveness of treatments, adjusting or making recommendations for changes to treatment, and making onward referrals.
