Job summary
An exciting opportunity has arisen for a Cardiac Rehabilitation Specialist, this role is suitable for a Nurse or Physiotherapist. The post holder will be a key member of the multi-disciplinary team in delivering a comprehensive cardiac rehabilitation programme for patients with cardiac conditions.
We will consider full time (37.5 hours to cover Powys) or part time/job share applications (18.75 hours to cover North Powys and 18.75 to cover Mid/South Powys).
Main duties of the job
To deliver a comprehensive, high quality evidenced based Cardiac Rehabilitation service in collaboration with the Multi-disciplinary Teams across Primary, Community and Secondary Care along integrated pathways of care in Powys.
Actively contribute to the planning and development of the Cardiac and Heart Failure services across Powys within the Community Cardiology Service.
This role will include delivery of exercise based cardiac rehabilitation in community settings across Powys, as well as patient assessment, medication review, appropriate follow up and evaluation of outcome measures.

- Degree in Nursing or Physiotherapy or equivalent experience
- Live registration with the NMC or HCPC as applicable
- Post registration specialist training / supplementary qualifications to Masters level in Cardiac/Heart Failure / rehabilitation or equivalent experience
- Independent Prescriber or willingness to commence Independent Prescriber training within 1 year
Desirable
- Teaching qualification / Teaching and Assessing in Clinical Practice or evidence of teaching skills/clinical educator
- MSc Advanced Clinical Practice
- Management/Leadership qualification or equivalent

- Extensive previous clinical experience of assessing, planning, implementing and evaluating care and rehabilitation of cardiac patients
- Comprehensive knowledge of exercise physiology and exercise prescription
