Wellbeing Practitioner - Single Point of Contact - Mental Health

Are you looking for an opportunity to play a pivotal role in Betsi Cadwaladr University Health Board's, first point of contact for patients accessing mental health services?

This is a pivotal role for individuals who access the Mental Health Single Point of Contact service, including individuals in distress, families/carers and professionals such as Police, GP's, A&E, WAST and Local Authority.

Using a recognised triage tool and compassionate focused interventions the post holder will assess the mental health needs of the individual using a range of communication methods adaptable to the needs of individuals, families/carers and partner organisations to review mental health telephone enquiries through assessing symptoms and health information, prioritising for urgency.

This role involves connecting individuals to the most appropriate mental health and well-being provision to meet their needs in a timely manner, including those with common mental health problems and those with more complex, acute and high-risk presentations. You will be required to liaise and communicate clinical information verbally and via technical links with health colleagues and partner organisations to ensure continuity of care for those accessing the service.

Main duties of the job

This is an exciting role which requires you to work autonomously, providing high quality clinical triage and risk management using knowledge, skills, critical thinking and professional judgement, supported by clinical triage protocols/policies and support from Senior Practitioner colleagues where needed. As first point of contact you will provide advice, guidance and support to partner organisations and professionals such as Police, WAST, A&E, GP's and Local Authorities on mental health related issues.
