An exciting opportunity has arisen in Primary Mental Health Services for an experienced, motivated and forward thinking practitioner to join a busy supportive team based in the West area.
Applicants with a professional registration in occupational therapy, social work and nursing are welcome to apply.
Position has arisen with the South Community Mental Health teams at an exciting time where services are under review and development in order to meet the changing needs of both patients and carers.
The successful candidate whilst working closely with a multi - disciplinary team, will receive referrals directly from GP's and provide short term interventions for individuals with mild to moderate illness and will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies.
The successful candidates will manage a caseload of patients with whom they will work alongside to formulate risk management plans and care planning based on robust assessment of need.
We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.
Main duties of the job
Provide initial mental health assessments in line with the agreed BCUHB assessment procedures for the LPMHSS.
Provide time-limited interventions in the primary care settings for patients with mild to moderate mental health problems.
Act as the first point of contact for GP's, to offer advice and support and signposting on to appropriate agencies. Manage their own therapeutic care/interventions within the clinical environment and be accountable for the quality of care they deliver within the clinical area that he or she is responsible for.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
