Job summary

We have an exciting opportunity in Wrexham CAMHS for a Pathway Lead for Transitions and Secondary Care. Our vision is to continue with the development of a more streamlined approach to practice across pathways.

The Pathway Lead for Transitions and Secondary Care in CAMHS will have a solid understanding of the Mental Health Measure Wales 2010. You will have experience of managing a caseload of young people with a range of mental health needs and supporting young people through their transition to adult mental health services. You will also will lead on the monitoring and delivery of this clinical pathways and provide leadership support to pathways that have interdependencies with transitions and secondary care.

In addition, you will have experience of being responsible for providing evidence-based assessments and therapeutic treatment for these individuals alongside supervising CAMHS Practitioners with casework.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

Pathway leads will ensure that a high quality, timely, accessible and evidence-based provision is available to all children and young people who need it. This means establishing and maintaining excellent working relationships with wider social, education, health and voluntary services.

Applicants should have a relevant professional and registered qualification relating to Health or Social Caree.g. in social work, mental health or learning disability nursing, or Occupational Therapy and a high level of competence, knowledge and skills (underpinned by knowledge of effective risk assessment and relevant evidence-based practice) which is crucial to this role.

The successful applicant must be flexible, adaptable, and have a willingness to embrace change as and when required. The ability to use initiative when dealing with unexpected situations is also an advantage. Applicants should be able to demonstrate evidence of their clinical understanding and decision-making skills; be a committed team player with the ability to prioritise their own workload, and work well under pressure. Good IT skills are essential with confidence in outlook, word and excel.
